COME PREPARED

Here are the steps that you can take to ensure that your visit at Manatee Gynecology is the most productive it can be:

  • FOR ALL NEW PATIENTS: visit our Patient Portal to fill out your Patient Demographic Information.
    Practice Code is: GBBCBD

    • Click “Pre-Register” Under “New to the Practice”

    • Please fill out ALL information

    • Once it is completed, our office will contact you to schedule your appointment

      • Please note: You must complete the form in ONE SITTING - If you click out of the portal, you will not be able to pick up where you left off.

 

If unable to complete paperwork through the portal, then please print, read, and fill out the New Patient Packet.
However, you do not need to complete it in two places.

Additionally, Please print and fill out “Patient Information", “Office Policies,” and “Privacy Policy” forms (linked below) so that you can bring it with you.

If you are a new patient, please arrive
30 minutes prior to your scheduled appointment time.

Please note, if you are late to your appointment time (as a new or existing patient),
there is a good chance you will have to be rescheduled.

If you do not show up to your appointment without 48 hour notice, you will be charged $25.
If you miss a scheduled procedure without proper notice, you will be charged $75.

The following items will be needed at every appointment:

Insurance card

Identification card

Current medication list (including any over-the-counter products)

Payment method for services provided

Medical records from other physicians — to make the most of your visit, it is important that we have your records prior to your appointment. We recommend calling our office in advance to confirm receipt of all necessary medical records.

Consults

Please note that a pelvic exam will be required prior to any procedure or surgery, including MonaLisa Touch treatments. This ensures we provide the best care and tailored solutions for your needs.

Mammogram

On the day of your mammogram, please don't apply deodorant, antiperspirant, powders, lotions, creams, or perfumes under your arms, or on or under your breasts. Particles from these may show up as white spots on your mammogram that cause confusion. If you are new to our facility, please bring your previous breast imaging reports and images on a CD to your appointment.

Ultrasound

Please check in at the front desk. Drink a minimum of 24 ounces of any fluid at least one hour before your appointment. Do not empty your bladder until you are instructed to do so by the Ultrasound technologist. No fasting or sedation is required for a pelvic ultrasound. Your pelvic ultrasound with also include internal imaging, where a small wand will be placed inside the vagina to see your uterus and ovaries. If you are having a pelvic ultrasound, you will typically have a visit imediately after to review the results with your provider. If you are having breast, kidney, or other organs imaged, those studies will be read by our radiologist and your provider will review results with you when they become available.

Bone Density Testing

When you arrive, please check in at the front desk. Please stop taking any calcium supplements 24 hours before the test. Wear comfortable clothing. You may have to take off clothes with metal fasteners, zippers, or hooks. Your Bone Density results will be reviewed with one of our nurse practitioners immediately following your test. You will be provided with a copy of your results for your records. Any previous testing from outside facilities is requested prior to your current testing date.
