Option 1: Patient Portal Access

You may also complete the questionnaire through our Patient Portal:

Click the blue link > PATIENT PORTAL

1. Enter Practice Code: GBBCBD

2. Select Manatee Gynecology

3. Choose "Login to Patient Portal Using Mobile Phone"

4. Enter your First Name, Last Name, and Date of Birth

5. Click Submit, then Send Code to your mobile

6. Enter the code and login

7. Locate your appointment on the dashboard

8. Click “Fill out Questionnaire” under your scheduled appointment

9. Complete and submit the form

Option 2: Text Message Link

1. You’ll receive a text message with a link to start the check-in process, which includes your required clinical questionnaire.
   This text is automatically sent 7 days and 3 days before your appointment.

• Arrive 30 minutes before your scheduled appointment time
  If you arrive late, you will be rescheduled.

• If you’re unable to access the portal, please download and print the New Patient Packet, fill it out completely, and bring it with you to your visit. You can find these forms under the “Come Prepared” Tab.
